Our objective was to examine management of mineral metabolism in patients wait-listed for renal transplant and to determine the impact (...) on cardiac perfusion imaging.Methods: Data was collected retrospectively on patients wait-listed for a renal transplant, being either active and on hold. Demographic data, medications, serum concentrations of calcium, phosphate, parathyroid hormone, and cardiac perfusion imaging studies were collected from the electronic health record. Chi-square and Student’s t-tests were used to compare active and on-hold patients as appropriate. Logistic regression was used to examine variables associated with worsening cardiac imaging studies.Results: The wait-listed patients were of mean age 56 ± 14 years and had been on dialysis for 1329 ± 867 days. On-hold patients had received a significantly greater total dose of calcium and were more likely to have developed worsening cardiovascular imaging studies. Total doses of calcium and calcitriol were associated with worsening cardiovascular imaging studies.Conclusion: Patients on hold on the renal transplant waiting list received higher total doses of calcium. A higher total dose of calcium and calcitriol was also associated with worsening cardiovascular imaging. Time on dialysis before transplant has been associated with worse post-transplant outcomes, and it is possible that the total calcium and calcitriol dose received contributed to these inferior outcomes.Keywords: dialysis, calcium, cardiac, transplantation. In _Poverty: a philosophical approach_, the author studies various philosophical issues concerning poverty in the Program for Education, Health and Food (PROGRESA) that was in effect in Mexico, from 1997 to 2002, and shows how theoretical discussion is necessary to clarify some ideas concerning the application of a social policy. Poverty is one of the main problems concerning economics, political philosophy, and ethics. It is an ethical problem because of its relationship with self-esteem. Since poverty is intimately related to social (...) policies, the philosophy of poverty must consider the distribution criteria used to attend to people in situations of extreme poverty. This would involve attention to their needs, preferences, capabilities and “well-being” rights. The book considers social policies applied to poverty, and their occasional abuse of utilitarian instruments. Many are implemented without considering cultural differences, including varying patterns of conduct in diverse communities. Equality also matters. Since poverty and inequality are not the same, the study of the latter allows us to target groups found in the lowest levels of “the playing field”. (shrink)

This article is a phenomenological study of students' lived experience in a computer conference learning environment. Issues for students in computer conferences revolved around spatial disorientation: existential location, appearances in space, imagined space, word space, secret space, and being alone within a group. Within these spatial issues the students grappled with getting into a computer space, familiarity of use, getting stuck, and the class as a "necessary evil. Alexander Pho and Benjamin A. White respond to Nicolas Dixon’s critique of mixed martial arts (MMA) through a ‘companions in innocence’ argument. Taking up a counterexample that Dixon is quick to dismiss, the authors argue that MMA techniques are on a par with the ‘pain-leveraging’ tactics used by cyclists and that pressing for a moral distinction between cycling and MMA leads to absurd conclusions about other practices. So, because cycling is morally permissible, MMA is morally permissible. This companions in innocence (...) argument fails. But cycling can be used to develop a better, though only partial, defense of MMA. (shrink)

The article focuses on motivation, proposing the equation in its title and opposing the contrary view, that what moves people to action is the rational calculation of their material interests. The latter view is most familiar in economics, where it was for generations seen as the best (meaning, most ‘scientific’) mode of explanation. It had a great deal of influence on historiography and found a great deal of support among psychologists also. From these three areas of research it is being (...) challenged, however, even to some extent displaced, in recent decades. The importance of emotions or feelings in explaining behavior is increasingly acknowledged, and the validity of conclusions reached about feelings in historical contexts through the operation of empathy. BackgroundA fundamental ethical challenge in conducting genomics research is the question of what and how individual level genetic findings and aggregate genomic results should be conveyed to research participants and communities. This is within the context of minimal guidance, policies, and experiences, particularly in Africa. The aim of this study was to explore the perspectives of key stakeholders' on returning genomics research results to participants in Kenya.MethodsThis qualitative study involved focus group discussions (FGDs) and in-depth interviews (IDIs) with 69 stakeholders. (...) The purposively selected participants, included research ethics committee (REC) members (8), community members (44), community resource persons (8), and researchers (9). A semi-structured interview guide was used to facilitate discussions. Six FGDs and twenty-five (IDIs) were conducted among the different stakeholders. The issues explored in the interviews included: (1) views on returning results, (2) kind of results to be returned, (3) value of returning results to participants, and (4) challenges anticipated in returning results to participants and communities. The interviews were audio-recorded, transcribed verbatim, and coded in Nvivo 12 pro. Thematic and content analysis was conducted.ResultsParticipants agreed on the importance of returning genomic results either as individual or aggregate results. The most cited reasons for returning of genomic results included recognizing participants' contribution to research, encouraging participation in future research, and increasing the awareness of scientific progress. Other aspects on how genomic research results should be shared included sharing easy to understand results in the shortest time possible and maintaining confidentiality when sharing sensitive results.ConclusionsThis study identified key stakeholders’ perspectives on returning genomic results at the individual and community levels in two urban informal settlements of Nairobi. The majority of the participants expect to receive feedback about their genomic results, and it is an obligation for researchers to see how to best fulfil it. (shrink)

I believe this development was influenced by my use of prescribed medications. Several neurologists (...) from Los Angeles to San Francisco directed various electroencephalograms (EEGs), including a week–long, prescription–free EEG in the hospital; magnetic resonance imaging tests (MRIs); computed tomography (CT) scans anywhere from forty–five minutes to four hours each; and blood profiles to help determine my situation. For legal reasons, one neurologist even had me sign a release stating that he was not able to diagnose my situation.Test results present limited information about my brain and without ever saying, “I don’t know,” these neurologists communicated my test results by utilizing technical terms and thus not always answering my questions. After explaining that my head turns to the left while my right hand shakes, symptoms that have been recognized and reported by several friends and family members, my doctor replied that a seizure on the left side of my brain should cause my head to turn to the right and then changed the subject. I felt like asking, “Am I doing it wrong?”The brain is different from other parts of our anatomy in that it can’t be replaced like a kidney, heart or other organ. It cannot be removed like tonsils, an appendix or even a limb. It can be physically studied or repaired while functioning but the resulting percentage of survival or proper operation of the body after surgery is minimal and the number of people requiring such attention is so low that the amount is scanty. A dead brain limits the analysis of its functions.My “disabilities” involve spontaneous uncontrolled loss of consciousness, exhaustion, mood swings, and the suspension of driving privileges. Activity interference, extra naps, losing friends and not being able to get around are definite disadvantages. However, everyone needs to manage personal problems. No one is perfect.Public transit schedules and directions don’t always offer me the transportation I need. Fortunately, the combination of a bus ride with walking or running can serve my needs while also helping me to keep physically healthy.Limited benefits include discounted transit and the option to ride instead of walking or running. This paper examines a legal case arising from a workplace grievance that progressed to being heard at the UK’s Supreme Court. The case of Doogan and Wood versus Greater Glasgow and Clyde Health Board concerned two senior midwives in Scotland, both practicing Roman Catholics, who exercised their perceived rights in accordance with section 4 of the Abortion Act not to participate in the treatment of women undergoing abortions. The key question raised by this case was: “Is Greater Glasgow and Clyde (...) Health Board entitled to require the midwives to delegate, supervise and support staff in the treatment of patients undergoing termination of pregnancy?” The ethical issues concerning conscientious objection to abortion have been much debated although the academic literature is mainly concerned with the position of medical practitioners rather than what the World Health Organization terms “mid-level professionals” such as midwives. This paper examines the arguments put forward by the midwives to justify their refusal to carry out tasks they felt contravened their legal right to make a conscientious objection. We then consider professional codes, UK legislation and church legislation. While the former are given strong weighting the latter was been ignored in this case, although cases in other European countries have been prevented from escalating to such a high level by the intervention of prominent church figures. The paper concludes by stating that the question put to the courts remains as yet unanswered but offers some recommendations for future policy making and research. (shrink)

Over the last decade, the c‐myb gene and its protein product, Myb, have undergone extensive examination and manipulation in hemopoietic tissues. Although it is rarely disputed that, as a transcription factor, Myb regulates cell cycling, proliferation and differentiation, identification of genes directly controlled by Myb has been surprisingly difficult. More recently, genes with promoter regions that contain Myb recognition sequences have been identified, but a direct proliferative response to Myb via these ‘target genes’ has yet to be demonstrated. Mutagenesis studies (...) have defined domains of the protein which influence its transcriptional activity and transforming potential; however how the molecule interacts with itself and with other cellular factors is only beginning to be understood. A broader examination of c‐myb expression in normal and malignant tissues suggests an analogous role for Myb in proliferation, differentiation and transformation of nonhemopoietic tissues. (shrink)

In an undogmatic, non-technical treatment, Steele contends that both the Marxian conception of future society and the Misesian argument against its feasibility have frequently been misunderstood. While abortion has been legal in most developed countries for many years, the topic remains controversial. A major area of controversy concerns women’s rights vis-a-vis the rights of health professionals to opt out of providing the service on conscience grounds. Although scholars from various disciplines have addressed this issue in the literature, there is a lack of empirical research on the topic. This paper provides a documentary analysis of three examples of conscientious objection on religious grounds to performing abortion-related care (...) by midwives in different Member States of the European Union, two of which have resulted in legal action. These examples show that as well as the laws of the respective countries and the European Union, professional and church law each played a part in the decisions made. However, support from both professional and religious sources was inconsistent both within and between the examples. The authors conclude that there is a need for clear guidelines at both local and pan-European level for health professionals and recommend a European-wide forum to develop and test them. (shrink)

The paper presents Aquinas’s account of conscience, and argues that key elements of this account are key elements too of Aristotle’s moral theory. The paper’s purpose is to encourage debate over conscience as not only a Stoic/Christian concept but one with deeper— and more widespread—roots in western ethical tradition.

This book tries to explain how decisions to act develop in the mind. Emphasis is on group decisions not only of the present but also from the past, where laboratory techniques can t apply. What emerges is a description of a process rather than the definition of a word. The description points to kinds of data that need special consideration: data regarding ideas of right and wrong, cultural traditions, emotional packaging.".

What does philosophy have to say about the argument that blasphemous art ought not to be publicly displayed? We examine four concepts of blasphemy: blasphemy as offence, attack on religion, attack on the sacred, attack on the blasphemer himself. We argue all four are needed to grasp this complex concept. We also argue for blasphemy as primarily a moral, not a religious concept. We then criticise four arguments for the public display of blasphemous art: it may be beautiful, provocative, devoutly (...) intended, and is autonomous of religious concerns. Finally, we discuss the notions of blasphemy and blasphemous art as public offences. We conclude that the display of blasphemous art is a public, and not merely a private moral offence, and that there are respectable philosophical arguments for this conclusion. (shrink)

The purist/partisan distinction has dominated recent discussions of sport spectatorship and sport aesthetics. The focus of such discussions, however, has been sport in general or, often implicitly, team sports in particular. Here, using mixed martial arts (MMA) as a case study, we argue that specific aspects of the sport in question can significantly affect how the purist/partisan distinction plays out for viewers. MMA’s status as an individual combat sport mitigates, in illuminating ways, the partisanship displayed so prominently among fans of (...) team sports. We also examine the role of external attitudes – in both the betting subculture of MMA and purist attitudes toward individual combat sports and martial arts – in shaping MMA spectatorship. (shrink)

In this paper, we discuss the ethical responsibility of the Information Technology (IT) industry towards its female workforce. Although the growing IT industry experiences skills shortages, there is a declining trend in the representation of women. The paper presents evidence that the IT industry is not gender-neutral and that it does little to promote or retain its female workforce. We urge that professional codes of ethics in IT should be revised to take into account the diverse needs of its staff.

This is the first book to pay MMA the serious philosophical attention it deserves. The book explores topics such as whether MMA qualifies as a martial art, the differences between MMA and the traditional martial arts, the aesthetic dimensions of MMA, the limits of consent and choice in MMA and whether MMA can promote moral virtues.
